Once completed, print and/or email the checklist to help you acquire all needed documentation before going to a Deputy Registrar Agency to obtain your driver license or identification card.
To prove identity, you must show proof of the following five elements
1-Full Legal Name*; 2-Date of Birth; 3-Proof of U.S. Legal Presence; 4-Social Security Number; 5-Proof of Address
To prove an element, the element must be contained on the document. One document from this list may be presented for a duplicate or renewal issuance (does not apply to Non U.S. citizens).

NOTE: Customers are encouraged to use this checklist to prepare in advance of visiting a Deputy Registrar agency. Use of this checklist does not guarantee issuance of a DL-ID. Issuance will be based on the validity and acceptance of actual documents presented at the time of the transaction.

Is your current full legal name the same as the name listed on your ?
If your current full legal name is not the same as your birth name, proof of name changes must be met.
Documents must be presented to connect your birth name to your current full legal name.
Below are a few examples of documents to bring with you to acquire your DL-ID.
*NOTE: If you have had more than one marriage, you may need to bring documentation from each marriage and/or divorce to connect your birth certificate, passport or U.S. Department of Homeland Security documents
to your current legal name. Permanent Resident Card (Form I-551) must show current legal name.
If your most recent USCIS document does not display your current legal name, you will need to update your document with the issuing agency to reflect your current legal name.
This includes name changes related to marriage/divorce, court ordered name changes, and all other name changes.
You may present an application to replace your USCIS document in order to show the current legal name, via an I-797 or I-797C with an acceptable notice type.
Your **Supplemental USCIS document(s) proving Legal Presence must display your current legal name, which will be the name displayed on your Ohio credential.
